Delayed Presentation of Traumatic Lumbosacral Dislocation: Report of a Rare Injury and a Comprehensive Review of Basic Principles of Management

Abstract

Introduction: Traumatic lumbar dislocation (TLD) is a rare clinical entity and is characterized by dislocation of facets of the lower lumbar vertebrae over the articulating facets of subjacent vertebrae. Case description: We hereby report two rare case scenarios of patients who presented with neglected TLD/traumatic lumbosacral dislocation (TLSD) injuries. The patients were managed surgically with laminectomy, vertebral reduction, and transforaminal lumbar interbody fusion (TLIF). Conclusion: These cases are highlighted for their rarity. We have comprehensively discussed the unique technical challenges faced by the surgical team and basic principles in the management of such injuries. TLD/TLSDs are rare injuries and can be safely managed surgically through wide decompression, complete reduction, and instrumented fusion using a posterior-only approach.
